**All Camps hosted on the ASU campus require the forms listed below for participation**

1. Release of Liability (Minor)
2. Release of Liability (19 and over)
3. Image Release
4. Medical Information Form
5. Food Allergies/Special Diets Form (If Applicable)

If you are participating in a residential camp, please remember to bring the following items:

1. Bed Linens (sleeping bags suggested)
2. Bath Linens (towels) 
3. Personal Toiletries
4. Shower Curtain & Rings
5. Trash bags

Please review the Residence Hall Guidelines before your stay.
