Health Evaluation Form Requirements:

All first-time students and incoming freshmen are required to submit a complete Health Evaluation Form package before enrollment. This package must include:

1. Page one of the Health Form (Completed by the student)
2. Proof of two MMR vaccines (Immunization Record)
3. Required TB test (Completed within the last 12 months)
4. Required physical exam (Completed within the last 12 months)

Failure to submit these documents may result in delays in receiving health services, registering for classes, and securing campus housing.
